Landing Page Design for Colon Cleanse Product

Help people cleanse their colons: Create landing page based on referenced landing page in the description. This particular style of landing page needed is known as a "reverse squeeze page" because it shows an informational video and then if they want more information, they need to put their name and email.

Another approach - here is different places to opt in (above the video and four different places in the body copy). But the design should be way way better than the Mass Control site which doesn't have much design.
