We do digital advertising across all therapeutic categories targeting multiple audiences. This project is in the cholesterol category targeting consumers.

Looking for animated (.gif, .jpg or HTML5) web ads in sizes: 728x90, 300x250, 320x50 and 160x600, that encourage users to complete a survey to see if they qualify for a clinical trial for a cholesterol medication. Looking for total of 16 banners - 4 sizes (sizes above) | 4 different designs User will go first to a landing page with a brief explainer video, then be asked to take a brief survey. The key to completing the survey is they need to "know their numbers" (or have easy access to them) in terms of their current cholesterol panel numbers.

No brand should be mentioned in the banners. Free to use any of the icons and or accents from the landing page (below) and/or style guide (attached).
