We need a logo to brand our new products: Navi.Care (Pro - web-base solution for patient navigators and Home - mobile app for patients).
Our team is dedicated to powering patient navigators at two levels- the level of the home caregiver (Navi.Care Home) and at the professional level (Navi.Care Pro)- community health workers, case managers, pharmacists or care coordinators, so that they can manage their patients’ out of clinic care plan.
Our team is well versed in healthcare, drug development and regulatory expertise, including my ten years in biomarkers and drug development as well as 3 years as the top science and innovation advisor to the FDA Commissioner. We use this experience along with intelligence gathered from patients and healthcare professionals to build a technology that is flexible and works for them.
The technology is predicated on a series of modules that addresses different facets of health outcomes management and research. This includes managing and tracking medication, vitals, patient education, and patient reported outcomes all connected on the cloud in a seamless ecosystem with business intelligence built in at each level.
The real power of the technology lies in providing an easy to use and scalable tool for the professional patient navigator. These navigators can build content, create their own surveys and questionnaires, manage workflows for each patient encounter and perform individual patient or population based analytics through a simple to use dashboard.
This tool is highly usable in management of community practice models that are being increasingly used for management of high risk Medicare, Medicaid and underserved populations, where major impediments to successful health interventions are mainly due to inability to capture, manage and coordinate health information in real time.
Additionally, this tool supports measurement of health interventions on a population level to support real world evidence gathering increasingly needed by pharmaceutical companies to provide evidence for payment and development of better next generation products.
At the level of the patient, a simple tool to manage their care plan, this interface can be shared with their home caregiver empowering the home caregiver to co-manage. At the enterprise level, the technology allows for communication to and from patients in real time in order to better manage their health or study an intervention in real time.
